1. Bluebook Express Service Agreement

Services are available to employers with a minimum of 10 employees for their noncommercial use. Employers enrolling with Healthcare Bluebook agree they (and their employees) will not use the Services in commercial use for disease management, utilization management, healthcare navigation services, telemedicine, on-site clinics, second surgical opinions, any other human resource/health and welfare programs, etc. Employer agrees that each employee will use their own unique username and password.

Services will commence within 1 week of Enrollment submission. Services are a web-based service that enables users to research specific healthcare procedures to determine the Fair Price parameter for such procedure in an area and identify fair price and/or high quality providers that perform the procedure.

Terms & Conditions:

Services are not available to competitors of Healthcare Bluebook.
Healthcare Bluebook has sole discretion whether to accept or decline the provision of Service to any particular entity or individual.
The term of this agreement is for one year from enrollment, however Healthcare Bluebook may terminate Services at any time. If Healthcare Bluebook terminates in the middle of a term, a pro rata refund shall be provided.
$2.25 per Employee per month (the “Bluebook Fees”).
Client must enroll all employees who have access to the Client's sponsored benefit plan, and may choose to enroll additional employees who do not have such access.
Annual Payment required at the time of Enrollment submission.
- Employers enrolling greater than 250 employees for the Services will have the option to split the Annual Payment into 12 equal monthly payments, of which the first installment is due at the time of enrollment submission.
- Upon enrollment, the Client agrees to Net 0 ("Due Upon Receipt") payment terms.
At the end of each annual term, the Services automatically renew unless you notify us in writing at least 60 days in advance that you want to cancel or do not want to auto renew. Otherwise, you authorize us (without notice to you, unless required by applicable law) to collect the then-applicable membership fee and any taxes, using any credit card we have on record for you.
The Bluebook Fees do not include any sales tax or any other kind of taxes now in force or imposed at a later date. All applicable taxes, if any, shall be the responsibility of Client and, as applicable, may be collected by Healthcare Bluebook.
Client can update their employee list at any time which will be updated by Bluebook monthly. Additional charges may apply in the event that the number of employees increases.
